Visit a licensed healthcare provider (MD, DO, DC, or NP) to receive a referral or prescription for "Medical Massage" or "Manual Therapy". Ensure it includes a diagnosis code (ICD-10) and the frequency of treatment (e.g., 2x per week for 6 weeks).
Book your first session with us. Bring your prescription, insurance claim number, and adjuster's contact information.
We will submit the required CMS-1500 forms and SOAP notes directly to your insurance company for reimbursement.
